They said they only do it 2-3 times a year so can't be that standard.

Who said that? I don't believe that it's that infrequent in CLark Co. Jail. Usually inmates cuff up at the cuff port. If they don't and want to fight, then it's on! There's one guy for each limb.
What they did with the blind fold (I've used spit masks before) is pretty cool.
