Default help!-care plans

Anybody in the medical field out there? I into my second month of clinicals for nursing. It has gone ok until the last few days. I have actual clinicals next week and a big test tommorow morning over elderly and care plans. I should'nt have waited until the last minute, but have about 5 chapters to read about care plans and I have a major headache (too much reading, also school, work, workout already today) plus not enough sleep for the last week. Any tips on how to figure out care plans? We have to actually do one next week the night before we see our first patient. I thought they would walk us thru it the first time, but apparently not. I have the basics down from notes: assessing, diagnosis, planning, implementing, and evaluating. But have'nt got a clue what to actual do when filling one out. Things such as interventions, nursing diagnosis statement, ect. Seems like they should teach you this stuff when you actually do it on the floor, but we have to know it before..
