Diet, exercise.

There's really no other better approach than that.
You need to make sure your diet is clean. And you need to exercise, cardio and weights or resistance training.
What are these 'back issues'?
What can you do and what can't you do?
'Back issues' is a very broad statement.

Can you run? Or is the jarring effect too much?
If so, do the elliptical machine or even a bike as your form of cardio. Can you squat, leg press, bench, do pull ups, dips, calf raises....etc. etc?
I'd imagine that there's a bunch of things that you CAN do. And...doing something is better than doing nothing.
