Since I don't have a spotter training to failure isn't really an option for me on BB bench press, and I don't generally aim for failure on other exercises either out of habit

To achieve a state of fatigue, avoid failure and increase volume. When your at the point of fatigue, your body will tell you. You will lose motivation to workout. The weights you lift will not increase from workout to workout, your joints may begin to hurt. When this occurs, back off the volume but keep the weights (intensity) up. If your muscles arent able to achieve fatigue then your not doing something right. Increase your volume. Lift heavy, in the range of 2-6, and try to increase weight every workout.
